NOV/DEC REGISTRATION

The Fall session will end Saturday, Nov. 8th and the Nov/Dec session will begin Monday, Nov. 10th. If your swimmer was registered for the Fall session, please complete the Membership Application (page 8 in the packet) and return it to the office with your payment by Nov. 3rd. For new swimmers, the following forms need to be completed and sent to the office with your payment by Nov. 3rd:
Membership Application
Medical History and Release Form
Code of Conduct
Meet Attendance Form (swimmers only)
Volunteer and Meet Requirement Form(swimmers only)
All the required forms are available in the link below. If you are not able to print the Nov/Dec Registration Packet, please email the PAAC office.
If you have a new swimmer, please contact the PAAC office to schedule an evaluation.
